  • Finity's in-house creative team designs and creates attention-grabbing member communications that adhere to your brand standards as well as state and federal guidelines. Additionally, our creative team is capable of creating anything you need from special projects to full-scale campaigns. 


  Dedicated, peer-based call center

  • Our call center is staffed with dedicated, full-time, bilingual employees trained to help members with any aspect of the rewards program. For issues outside the program, Finity can conduct warm transfers to your member help line.

Whole Person Health Assessment and Profile

Finity's Whole Person Health Assessment and Profile extends beyond a traditional health risk assessment to meet whole person needs.


Having a full picture of a member's health means going further than just biometrics, that's why the Whole Person Assessment and Profile (WPH-AP) asks members about:

 

  • Social Determinants of Health (SDOH)
  • COVID Risks
  • Readiness to Change
  • Financial and Employment Risks
  • Health, Wellness, and Behavioral Risks


After completing the assessment, members are provided with instant condition-specific and SDOH resources, and connected to support based on their personalized, easy-to-understand assessment output.

As members input their information, plans receive deep analytic insights into the needs of their members on a daily basis.

The Whole Person Assessment is available as a standalone HRA solution, but can also be integrated into our rewards and engagement platform. Either way you choose, it's built on NCQA standards, and can be easily customized to meet state & plan requirements.
